As in previous years, we are again offering the option to sign up for advance notification of the June 2009 meeting, to better ensure your successful registration. If you would like this advance notice, please complete the secure advance notification form.

2008 Registration Fee:
$195 – Early Registration – must be completed on or before May 30, 2008.
$255 – Late Registration – completed after May 30, 2008.
The registration fee includes all course materials, breakfasts, breaks and snacks on Friday and Saturday, and lunch and dinner on Friday. You may bring a guest to the Friday night dinner for an additional $20. Select this option on the registration form.

Advance registration must be accompanied by payment in full. Funds payable in U.S. dollars. All online and fax registrations must include the number of a major credit card that will be charged for the registration fees.

Mailed registrations may include payment-in-full by check made payable to Annenberg Center for Health Sciences. Purchase orders are accepted with mailed registrations.
