intended audience

This activity was developed for physicians, nurse practitioners, nurses, and other health care providers treating patients with Type 2 diabetes.

statement of need

For patients with diabetes or hyperglycemia, any hospitalization can present countless hazards. There are concerns about how health care clinicians can best care for the patient entering the hospital, particularly in an emergency situation. There are questions about how health care professionals optimally manage blood glucose levels in patients whose scheduling and diet are disrupted. Hospital staff also face difficulty in transitioning inpatients with diabetes and hyperglycemia to outpatient management. Potentially compounding these issues is the level of communication among the various physicians, nurses, and other health care providers who have a say in the patient's care. This activity will address the issues arising at patient transitions to assist in optimizing the patient's long-term care and well-being.

 

learning objectives

Upon completion of this activity, participants should be better able to:

Describe the growing problem of diabetes and hyperglycemia and their impact on patients
entering the hospital
Outline assessment strategies and initial inpatient glycemic management for the newly
hospitalized patient
Discuss the use of insulin and nutritional support for patients with diabetes and hyperglycemia
within the hospital to ensure safe transitions from criticalcare to other inpatient settings
Determine educational and discharge needs of patients and their families as well as improve
communication with outpatient care providers

The Annenberg Center for Health Sciences at Eisenhower is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

The Annenberg Center designates this educational activity for a maximum of 3.5 AMA PRA Category 1 Credits tm. Physicians should only claim credit commensurate with the extent of their participation in the activity.

Annenberg Center for Health Sciences is approved as a provider of nurse practitioner continuing education by the American Academy of Nurse Practitioners. Provider # 040207. The Annenberg Center designates this program for 3.5 contact hours of continuing education.

Annenberg Center for Health Sciences is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.
A maximum of 3.5 contact hours may be earned for successful completion of this activity.

There is no charge for this activity. Statements of Credit will be provided by mail following activity participation, and upon completion and return of the evaluation form to the Annenberg Center for Health Sciences. Please allow 4-6 weeks for the delivery of your statement.
